Technetium Tc-99m tin colloid + isosulfan blue is a dual-tracer diagnostic combination used for sentinel lymph node (SLN) mapping and biopsy, primarily in patients with breast cancer. This technique utilizes the complementary properties of a radioactive tracer and a visual dye to improve the accuracy of SLN identification. Tc-99m tin colloid is a radiopharmaceutical that allows for preoperative lymphoscintigraphy and intraoperative localization using a gamma probe; the colloid particles are transported via lymphatics and trapped by macrophages in the sentinel nodes. Isosulfan blue is a triarylmethane dye that binds to interstitial albumin and provides a visual blue map of the lymphatic channels and nodes during surgery. The combination of these two agents significantly increases the detection rate of sentinel nodes compared to using either agent alone, facilitating the assessment of metastatic spread to the axillary lymph nodes.
