Overview

Technetium Tc 99m TRODAT-1 is a technetium-99m-labeled tropane derivative radiopharmaceutical used as a diagnostic imaging agent in single-photon emission computed tomography (SPECT). It acts as a high-affinity ligand for the presynaptic dopamine transporter (DAT), also known as SLC6A3, located in the striatal region of the brain. By binding to DAT, the drug allows for the visualization and quantification of functional dopaminergic neuron terminals, which is critical for the differential diagnosis of Parkinsonian syndromes (such as Parkinson's disease, multiple system atrophy, and progressive supranuclear palsy) from other conditions like essential tremor. Developed primarily by the Institute of Nuclear Energy Research (INER) in Taiwan and researchers at the University of Pennsylvania, it serves as a more cost-effective and widely available alternative to iodine-123-labeled agents (such as Ioflupane I 123), leveraging the optimal 140 keV gamma emission and logistical advantages of the technetium-99m isotope.
