Tegoprubart is a humanized monoclonal antibody that targets the CD40 ligand (CD40L, also known as CD154), a key costimulatory molecule involved in both innate and adaptive immune responses. By binding to CD40L, tegoprubart blocks its interaction with the CD40 receptor on antigen-presenting cells such as B cells and dendritic cells, thereby inhibiting T cell activation and downstream pro-inflammatory signaling. This mechanism is designed to suppress immune-mediated damage while promoting a more tolerogenic environment. Tegoprubart is being developed primarily for the prevention of organ transplant rejection (notably kidney transplantation) and for neuroinflammatory diseases such as amyotrophic lateral sclerosis (ALS). The drug was originally discovered by ALS Therapy Development Institute, further developed by Anelixis Therapeutics, and is now under development by Eledon Pharmaceuticals following acquisition. It has completed Phase 1 trials in ALS and is currently in Phase 2 clinical trials for kidney transplant rejection[1][2][5][6][7][8].
