The combination of telbivudine and adefovir dipivoxil is used for treating chronic hepatitis B virus (HBV) infection. This combination therapy has shown particular efficacy in patients with resistance to adefovir dipivoxil alone and has demonstrated beneficial effects on renal function compared to other combination therapies. ## Pharmacological Properties Telbivudine and adefovir dipivoxil are both nucleotide/nucleoside analogs that work by inhibiting HBV DNA polymerase, but they do so through slightly different mechanisms. Telbivudine is metabolized to its 5-triphosphate derivative, which inhibits viral replication primarily by interfering with DNA-to-DNA transcription and causing chain termination[6]. Adefovir dipivoxil is a prodrug that is converted to adefovir diphosphate, which competes with the natural substrate deoxyadenosine triphosphate and causes DNA chain termination after incorporation into viral DNA[4]. Studies have shown that these drugs do not significantly interact with each other pharmacokinetically. When administered together, neither drug appears to alter the steady-state plasma pharmacokinetics of the other[3]. This pharmacological compatibility provides support for their use in combination therapy. ## Clinical Efficacy The combination of telbivudine and adefovir dipivoxil has demonstrated significant clinical benefits: 1. **Improved Viral Suppression**: In HBeAg-positive chronic hepatitis B patients with resistance to adefovir dipivoxil, the combination therapy led to significant decreases in serum HBV DNA levels[2]. 2. **Higher HBeAg Seroconversion**: The combination therapy exhibited a significantly higher rate of HBeAg seroconversion (20%) compared with entecavir monotherapy (0%) in patients with adefovir resistance[2]. 3. **Renal Function Benefits**: Unlike other adefovir-based combination therapies, telbivudine + adefovir dipivoxil has been shown to improve estimated glomerular filtration rate (eGFR) levels. After 104 weeks of therapy, patients receiving this combination showed a significant increase in eGFR from baseline (104.1 mL/min/1.73 m² to 116.2 mL/min/1.73 m²)[1]. 4. **Lower Risk of Renal Impairment**: Compared to entecavir + adefovir and lamivudine + adefovir combinations, the telbivudine + adefovir combination was not identified as an independent risk factor for reduction in baseline eGFR[1]. ## Safety Profile The combination therapy is generally well-tolerated. In clinical studies, no serious side effects were observed in the majority of patients[2]. However, as with any antiviral therapy, monitoring for potential adverse effects is recommended. The improved renal function observed with telbivudine + adefovir dipivoxil makes this combination particularly suitable for patients with potential renal impairment[1], which is a significant consideration in long-term hepatitis B treatment. ## Resistance Profile While resistance can develop with both drugs individually, the combination therapy may help mitigate this risk. In one study, only one patient receiving telbivudine + adefovir had viral breakthrough with a telbivudine-associated resistance mutation (rtM204I) detected during 48 weeks of treatment[2]. This suggests that the combination may provide a higher barrier to resistance than monotherapy with either agent. The combination of telbivudine + adefovir dipivoxil represents an important treatment option for chronic hepatitis B, particularly for patients with adefovir resistance or those at risk for renal impairment.
