Overview
Telcagepant is a small molecule antagonist of the calcitonin gene-related peptide (CGRP) receptor. It was developed by Merck & Co for the acute treatment and prevention of migraine. CGRP and its receptors are found in areas of the central and peripheral nervous system that are important for transmitting migraine pain. By blocking CGRP from binding to its receptor, telcagepant inhibits pain signal transmission associated with migraines. Telcagepant demonstrated efficacy comparable to triptans such as rizatriptan and zolmitriptan in clinical trials for acute migraine treatment. However, development was discontinued due to concerns about liver toxicity after some patients experienced significant elevations in liver enzymes during clinical studies[1][2][3][5].
