telmisartan + bisoprolol is a combination therapy consisting of an angiotensin II receptor blocker (ARB) and a selective beta-1 adrenergic receptor blocker. This specific combination was evaluated by the Medical University of Vienna in the PONTIAC (NT-proBNP Guided Prevention of Cardiovascular Events in Diabetic Patients) clinical trial. The therapeutic strategy involves using these two agents to provide intensified cardiac prevention care in patients with diabetes mellitus who exhibit elevated NT-proBNP levels but have no prior history of cardiac disease. Telmisartan functions by selectively blocking the binding of angiotensin II to the AT1 receptor, leading to vasodilation and reduced aldosterone secretion. Bisoprolol is a cardioselective beta-blocker that reduces heart rate and myocardial contractility by inhibiting beta-1 adrenergic receptors. The goal of the combination in this context is to reduce cardiac workload and prevent the progression to overt cardiovascular events.
