Telpegfilgrastim (Paijin) is a long-acting recombinant human granulocyte colony-stimulating factor (rhG-CSF) developed by Xiamen Tebao Bioengineering. It features a unique 40 kDa Y-shaped branched polyethylene glycol (PEG) moiety conjugated to the N-terminus of rhG-CSF. This modification extends the drug's half-life by reducing renal clearance and protecting the protein from proteolytic degradation, allowing for less frequent dosing compared to short-acting filgrastim. Telpegfilgrastim functions as an agonist of the G-CSF receptor (CSF3R), stimulating the proliferation, differentiation, and activation of neutrophil precursors in the bone marrow. It is approved in China for the prevention and treatment of chemotherapy-induced neutropenia (CIN) in patients with non-myeloid malignancies, helping to reduce the incidence of febrile neutropenia and associated infections.
