Telratolimod (MEDI9197) is a small molecule imidazoquinoline agonist of toll-like receptor 7 and 8 (TLR7/8), designed for intratumoral injection to stimulate both innate and adaptive immune responses against cancer. By activating TLR7 and TLR8 on antigen-presenting cells such as dendritic cells and monocytes, it induces the production of proinflammatory cytokines (including type I interferons and interleukin-12), leading to recruitment and activation of effector immune cells like natural killer (NK) cells and cytotoxic T lymphocytes. This results in enhanced anti-tumor immunity, tumor cell lysis, modulation of the tumor microenvironment toward a Th1 phenotype, increased CD8+ infiltration, reduced CD4+ infiltration, and improved efficacy when combined with other immunotherapies such as PD-L1 inhibitors. The drug was developed by MedImmune for use in solid tumors via intratumoral administration to maximize local effects while minimizing systemic toxicity[1][2][4][5][6][7].
