This combination therapy consists of the alkylating agent temozolomide, the platinum-based chemotherapy cisplatin, and the PD-1 checkpoint inhibitor nivolumab. It is being investigated in a Phase II clinical trial led by Memorial Sloan Kettering Cancer Center for the treatment of advanced or metastatic mismatch repair-proficient (MMR-proficient) colorectal adenocarcinoma. Cisplatin works by forming DNA cross-links, temozolomide is an oral alkylating agent that causes DNA damage, and nivolumab is a monoclonal antibody that blocks the PD-1 receptor on T cells, enhancing the immune response against tumor cells. The combination aims to overcome the traditional resistance of MMR-proficient colorectal cancer to immunotherapy by using chemotherapy to potentially increase tumor immunogenicity and alter the tumor microenvironment.
