Temozolomide + sargramostim + aldesleukin + interferon alfa-2b is a combination chemobiotherapy regimen evaluated in a Phase II clinical trial for the treatment of unresectable stage IV metastatic melanoma. The regimen was developed and studied at the Northern California Melanoma Center at Saint Francis Memorial Hospital. It consists of temozolomide, an oral alkylating agent capable of crossing the blood-brain barrier, followed by a subcutaneous biological cocktail of granulocyte-macrophage colony-stimulating factor (GM-CSF/sargramostim), low-dose interleukin-2 (IL-2/aldesleukin), and interferon alfa-2b. This outpatient approach aims to combine the direct anti-tumor activity of chemotherapy with the systemic immune-potentiating effects of multiple cytokines to improve outcomes in patients with advanced melanoma, including those with M1c disease.
