Overview

Temozolomide + vinorelbine is an investigational combination chemotherapy regimen consisting of temozolomide, an oral alkylating agent, and vinorelbine, a semi-synthetic vinca alkaloid. This combination has been studied primarily for the treatment of recurrent or progressive brain metastases and high-grade gliomas. Temozolomide acts by methylating DNA at the O6 and N7 positions of guanine residues, leading to DNA damage and apoptosis in rapidly dividing tumor cells. Vinorelbine disrupts microtubule assembly by binding to tubulin, inhibiting mitosis at metaphase and promoting apoptosis in cancer cells. The rationale for combining these agents is based on their complementary mechanisms—DNA damage induction (temozolomide) with inhibition of cell division (vinorelbine)—and their ability to cross the blood-brain barrier[1][2][3][4]. Clinical studies have shown that this regimen is generally well tolerated with manageable hematologic toxicity.
