Temozolomide + vismodegib is an investigational combination therapy consisting of two small molecule drugs: temozolomide, an oral alkylating agent that induces DNA damage and apoptosis in tumor cells, and vismodegib, a selective inhibitor of the Hedgehog signaling pathway through antagonism of the Smoothened (SMO) receptor. This combination has been studied primarily for recurrent or refractory medulloblastoma with Sonic Hedgehog (SHH) pathway activation. The rationale is to target tumor proliferation at different molecular levels—temozolomide as cytotoxic chemotherapy and vismodegib as targeted therapy against SHH-driven oncogenesis. Clinical trials have shown that while the addition of vismodegib to temozolomide does not increase toxicity, it did not significantly improve progression-free survival compared to temozolomide alone in this setting[1][5][6][8]. Preclinical studies also suggest potential synergistic effects in glioblastoma models[2].
