Temozolomide + vistusertib is an investigational combination therapy consisting of two small molecule drugs. Temozolomide is an oral alkylating agent used primarily in the treatment of glioblastoma multiforme and refractory anaplastic astrocytoma. It acts by methylating DNA at the O6 and N7 positions of guanine residues, leading to DNA damage and apoptosis in tumor cells with impaired DNA repair mechanisms[5]. Vistusertib (AZD2014) is a dual inhibitor of mammalian target of rapamycin complexes 1 and 2 (mTORC1/2), targeting a key pathway involved in cell growth, proliferation, survival, and resistance to therapy[3][2]. The combination has been studied in early-phase clinical trials for relapsed/refractory malignancies including gliomas and sarcomas in both pediatric and adult populations. While generally well tolerated with manageable hematological toxicity as the main adverse effect[1][3], clinical benefit has been limited; most patients experienced stable disease or no response[1][3]. The rationale for this combination is based on preclinical evidence that mTOR pathway inhibition may sensitize tumors to alkylating agents like temozolomide.
