Overview

Temsirolimus + androgen deprivation therapy (ADT) is a combination therapeutic regimen investigated by the National Cancer Institute (NCI) for the treatment of prostate cancer, specifically in patients with biochemical relapse after local therapy. The regimen combines temsirolimus, an intravenous mTOR inhibitor, with various forms of androgen ablation, including LHRH agonists (such as leuprolide or goserelin) and antiandrogens (such as bicalutamide, nilutamide, or flutamide). Temsirolimus works by binding to FKBP-12 and inhibiting the mammalian target of rapamycin (mTOR) kinase, thereby blocking downstream signaling pathways involved in cell cycle progression and angiogenesis. ADT serves to deplete or block the action of androgens, which are critical for the growth of prostate cancer cells. The combination is designed to target two distinct but cross-talking pathways—the PI3K/Akt/mTOR pathway and the androgen receptor pathway—to overcome resistance and enhance clinical efficacy.
