Temsirolimus + liposomal doxorubicin is a combination therapy investigated primarily for the treatment of recurrent and refractory bone and soft tissue sarcomas. Temsirolimus is an intravenous inhibitor of the mechanistic target of rapamycin (mTOR), rapidly converted in vivo to sirolimus, which inhibits cell growth, proliferation, and survival by blocking mTOR signaling pathways. Liposomal doxorubicin is a formulation of the anthracycline chemotherapy agent doxorubicin encapsulated in pegylated liposomes, enhancing tumor localization and reducing toxicity compared to conventional formulations. Doxorubicin acts mainly by intercalating into DNA and inhibiting topoisomerase II, leading to DNA damage and apoptosis in cancer cells. The combination aims to sensitize resistant cancer stem cell populations (notably ALDH high sarcoma cells) to chemotherapy while maintaining tolerability for heavily pretreated patients[1][2][6][8].
