Tenecteplase is a genetically engineered, recombinant form of human tissue plasminogen activator (tPA) designed for single-bolus intravenous administration. It is a 527-amino acid glycoprotein produced in Chinese Hamster Ovary cells, with three specific amino acid substitutions that enhance its pharmacological properties: T103N and N117Q in the kringle 1 domain (affecting glycosylation and half-life), and K296A/H297A/R298A/R299A in the protease domain (increasing fibrin specificity and resistance to inhibition by PAI-1). Tenecteplase acts as a thrombolytic agent by binding to fibrin-rich clots, converting plasminogen to plasmin, which then degrades the fibrin matrix of thrombi. It is primarily indicated for reducing mortality associated with acute myocardial infarction (STEMI) but is also used off-label or investigationally for pulmonary embolism and acute ischemic stroke[1][2][3][5][6].
