Tenecteplase + butylphthalide is a combination therapy being investigated for the treatment of acute ischemic stroke, specifically to extend the intravenous thrombolysis window to 4.5–6 hours after symptom onset. This regimen is the focus of the EXIT-BT (EXtending the tIme Window of Thrombolysis by ButyphThalide) clinical trial series led by the General Hospital of Northern Theater Command (formerly General Hospital of Shenyang Military Region). Tenecteplase is a recombinant fibrin-specific plasminogen activator that promotes thrombolysis by converting plasminogen to plasmin. Butylphthalide (DL-3-n-butylphthalide) is a synthetic small molecule, originally isolated from celery seeds, that exerts neuroprotective effects by improving regional cerebral blood flow, protecting mitochondrial function, and reducing neuronal apoptosis and inflammation. The combination aims to provide both rapid recanalization and neuroprotection to salvage the ischemic penumbra in an extended time window.
