Overview

**Tenecteplase + heparin** is a combination therapy using tenecteplase, a genetically engineered variant of tissue plasminogen activator (tPA), with unfractionated heparin or low-molecular-weight heparin for acute pharmacological reperfusion. Tenecteplase acts as a fibrinolytic by specifically catalyzing the conversion of plasminogen to plasmin, facilitating the breakdown of fibrin in blood clots. Compared to alteplase, tenecteplase has a longer half-life and enhanced fibrin specificity, allowing single bolus administration. Heparin is an anticoagulant that potentiates antithrombin III to inhibit thrombin and factor Xa, reducing new clot formation. The combination aims to achieve rapid thrombolysis and prevent rethrombosis, especially during management of acute myocardial infarction, pulmonary embolism, or refractory thrombosis in severe COVID-19 and acute ischemic stroke settings. The regimen's safety profile requires close monitoring for hemorrhagic complications, particularly in elderly patients or those with elevated bleeding risk[1][2][3][4].
