Tenivastatin is a small molecule statin, specifically the active hydroxy acid metabolite of simvastatin. It acts as a potent inhibitor of HMG-CoA reductase, the rate-limiting enzyme in cholesterol biosynthesis. By inhibiting this enzyme, tenivastatin reduces hepatic cholesterol synthesis, leading to decreased levels of low-density lipoprotein (LDL) cholesterol and triglycerides while increasing high-density lipoprotein (HDL) cholesterol in the blood.
