Overview

Tenofovir alafenamide + boosted protease inhibitor refers to a therapeutic combination evaluated in a Phase 4 clinical trial (NCT02786589) conducted by researchers at the University of Colorado Anschutz Medical Campus. This study investigated the pharmacokinetics and renal safety of tenofovir alafenamide (TAF) when co-administered with a protease inhibitor (PI) that is pharmacokinetically 'boosted' by either ritonavir or cobicistat. TAF is a nucleotide reverse transcriptase inhibitor (NRTI) prodrug that inhibits HIV-1 reverse transcriptase, preventing viral DNA synthesis. Protease inhibitors block the HIV protease enzyme, which is essential for the cleavage of polyproteins into functional viral proteins, thereby preventing the maturation of infectious virions. The boosting agents, ritonavir and cobicistat, are potent inhibitors of the CYP3A4 enzyme, used to increase the systemic exposure and half-life of the protease inhibitor. The University of Colorado study specifically assessed whether the addition of the hepatitis C virus (HCV) treatment ledipasvir/sofosbuvir (Harvoni) significantly altered tenofovir exposures in HIV-positive patients already on a TAF-based regimen with a boosted PI, concluding that the combination was safe and did not lead to clinically significant increases in tenofovir levels.
