Tenofovir alafenamide + cobicistat is a fixed-dose combination of two small molecule drugs used primarily in the treatment of human immunodeficiency virus (HIV) infection. Tenofovir alafenamide is a nucleotide reverse transcriptase inhibitor (NRTI) that inhibits HIV-1 reverse transcriptase, thereby blocking viral replication. Cobicistat acts as a pharmacokinetic enhancer by inhibiting cytochrome P450 3A (CYP3A) enzymes, which increases the systemic exposure and effectiveness of coadministered antiretroviral agents metabolized by CYP3A, such as tenofovir alafenamide or other HIV medications. Cobicistat itself has no direct anti-HIV activity but boosts the levels of other antiretrovirals[6][8]. This combination may be found as part of larger multi-drug regimens for HIV therapy.
