The 2PR (two-pill regimen) is a combination therapy used as the control arm in clinical trials conducted by the HIV Prevention Trials Network (HPTN), such as PC952 and PC953. It consists of two separate daily oral tablets: Truvada (a fixed-dose combination of tenofovir disoproxil fumarate and emtricitabine) for HIV pre-exposure prophylaxis (PrEP) and a combined oral contraceptive containing ethinyl estradiol and levonorgestrel. The regimen is intended for women at risk of both HIV infection and unintended pregnancy. Tenofovir and emtricitabine function as nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs) to prevent HIV replication, while ethinyl estradiol and levonorgestrel act as hormonal contraceptives to prevent ovulation and pregnancy. The trials evaluate whether a single-pill Dual Prevention Pill (DPP) improves adherence compared to this standard two-pill approach.
