Overview

Tenofovir and entecavir are the primary nucleos(t)ide analog (NA) therapies used for the long-term treatment of chronic hepatitis B virus (HBV) infection. Tenofovir, a nucleotide analog available as tenofovir disoproxil fumarate (TDF) or tenofovir alafenamide (TAF), and entecavir, a guanosine nucleoside analog, both function by inhibiting the HBV DNA polymerase. By targeting the reverse transcriptase activity of the polymerase, these agents block the synthesis of viral DNA, effectively suppressing viral replication and reducing the risk of liver disease progression. In clinical trials for novel HBV therapies, such as the IAP antagonist birinapant, these agents are frequently employed as background standard-of-care therapy to maintain viral suppression while the experimental treatment is evaluated for its ability to achieve a functional cure (HBsAg loss).
