This combination therapy involves the use of parathyroid hormone (specifically the recombinant human PTH 1-34 fragment, teriparatide) and Granulocyte Colony-Stimulating Factor (G-CSF, filgrastim) to enhance hematopoietic stem cell (HSC) mobilization. Developed and studied by researchers at Massachusetts General Hospital, the regimen is designed for patients with hematologic malignancies such as lymphoma, multiple myeloma, and acute myeloid leukemia (AML) who have previously failed standard mobilization attempts. Teriparatide acts as an agonist at the parathyroid hormone receptor 1 (PTH1R), stimulating osteoblasts in the bone marrow to alter the HSC niche and promote the release of stem cells into the peripheral blood. Filgrastim acts as an agonist at the granulocyte colony-stimulating factor receptor (G-CSFR), stimulating the proliferation and differentiation of granulocytes and the release of HSCs from the bone marrow.
