Overview
TerXT is a next-generation, long-acting prodrug combination of xanomeline and trospium developed by Terran Biosciences for the treatment of schizophrenia. It is designed to overcome the limitations of existing formulations by enabling once-daily oral administration (TerXT) and multi-month injectable options (TerXT LAI). The prodrug approach optimizes pharmacokinetics, allowing for fixed-dose combinations that improve patient adherence and broaden access. Xanomeline acts as a muscarinic receptor agonist, while trospium is a peripherally restricted anticholinergic that mitigates peripheral side effects. This combination targets central muscarinic receptors to reduce symptoms of schizophrenia without causing significant extrapyramidal or metabolic side effects. TerXT leverages extensive medicinal chemistry optimization and preclinical testing to achieve its improved profile over earlier formulations[6][8].
