TerXT LAI is a long-acting injectable (LAI) prodrug formulation of the xanomeline and trospium combination, currently under development by Terran Biosciences (a Turret Capital Management portfolio company) for the treatment of schizophrenia. It utilizes the same pharmacology as KarXT (xanomeline/trospium), acting as a dual M1/M4 muscarinic receptor agonist to address psychotic symptoms while employing trospium, a peripheral antimuscarinic, to mitigate systemic side effects. TerXT LAI is designed to improve patient compliance and provide sustained therapeutic levels compared to oral formulations. It is currently in the preclinical/IND-enabling stage, pursuing an accelerated 505(b)(2) regulatory pathway.
