Testosterone cypionate is a synthetic derivative of the natural androgen hormone testosterone, formulated as an oil-soluble 17 (beta)-cyclopentylpropionate ester. It acts as a long-lasting prodrug of testosterone, providing sustained release after intramuscular injection. Its primary mechanism is agonism at the androgen receptor, mimicking endogenous androgens to promote development and maintenance of male sex characteristics and anabolic effects. It is FDA-approved for use in males with conditions associated with deficiency or absence of endogenous testosterone, such as primary hypogonadism (testicular failure) and hypogonadotropic hypogonadism (pituitary or hypothalamic dysfunction). Off-label uses include delayed puberty in boys, certain breast disorders in women, transgender hormone therapy for transmasculine individuals, oligospermia, and osteoporosis[1][2][5][7].
