Testosterone enanthate is a synthetic derivative of the naturally occurring androgen, testosterone. It is an esterified form (enanthic acid ester) that acts as a long-lasting prodrug of testosterone, providing sustained release after intramuscular or subcutaneous injection. Testosterone enanthate functions as an agonist at the androgen receptor, mediating its effects by promoting the development and maintenance of male secondary sexual characteristics, muscle and bone growth, libido, and spermatogenesis. In men and boys, it is primarily used for replacement therapy in conditions associated with deficiency or absence of endogenous testosterone such as primary hypogonadism (testicular failure), hypogonadotropic hypogonadism (pituitary/hypothalamic injury), and delayed puberty not due to pathological disorders. In women, it may be used for advanced inoperable metastatic breast cancer that is hormone-responsive[1][2][3][5]. It has also been used in masculinizing hormone therapy for transgender men[5]. The drug was first introduced into medical use in 1954.
