Overview

The tetracycline + furazolidone combination is a dual antibiotic regimen primarily used for Helicobacter pylori (H. pylori) eradication. This combination is often administered as part of a quadruple therapy that includes a proton pump inhibitor (like omeprazole or rabeprazole) and sometimes bismuth compounds. ## Mechanism of Action This combination works through complementary mechanisms: **Tetracycline** inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, preventing the attachment of aminoacyl-tRNA to the mRNA-ribosome complex. It passively diffuses through porin channels in bacterial membranes and may also alter bacterial cytoplasmic membranes, causing leakage of intracellular contents. **Furazolidone** is a synthetic nitrofuran with broad-spectrum activity against gram-positive and gram-negative bacteria as well as some protozoa. It works through multiple mechanisms: - Inhibits bacterial enzymes, particularly those involved in the Krebs cycle - Binds to bacterial DNA and induces cross-links, causing mutations in bacterial chromosomes - Undergoes nitro-reduction to form reactive metabolites that contribute to its antimicrobial activity ## Clinical Efficacy The combination has shown impressive efficacy in H. pylori eradication: - A 14-day tetracycline and furazolidone-containing quadruple regimen achieved 91.74% eradication rate (intention-to-treat) and 95.24% (per protocol) as rescue treatment - A 7-day triple therapy with omeprazole, tetracycline, and furazolidone showed 83.3% eradication rate (intention-to-treat) and 89.7% (per protocol) in pediatric patients This combination is particularly valuable because resistance to both tetracycline and furazolidone is rare, making it effective even when other antibiotics like clarithromycin, metronidazole, and quinolones have failed. ## Dosing Regimens Several dosing regimens have been studied: - 14-day quadruple therapy: rabeprazole 20mg twice daily + tetracycline 750mg twice daily + furazolidone 100mg twice daily + colloidal bismuth subcitrate 200mg twice daily - 7-day triple therapy: omeprazole once daily + tetracycline (or doxycycline) twice daily + furazolidone twice daily - 10-day pediatric regimen: omeprazole 20mg once daily + furazolidone 100mg twice daily + tetracycline 50mg/kg/day four times daily (or doxycycline 50mg twice daily) ## Side Effects Common side effects include: - Abdominal pain (30.5%) - Nausea (19.4%) - Vomiting (13.9%) - Fever, palpitation, and skin rash Furazolidone has monoamine oxidase (MAO) inhibitory effects and should not be given to patients already taking MAO inhibitors. It can also cause hemolysis in G6PDH-deficient patients. ## Cost Considerations The tetracycline + furazolidone combination is noted as a low-cost alternative for H. pylori eradication therapy. One study reported the total cost of this regimen was approximately US$11.14 per patient (maximum dose).
