Overview

Tetrahydrobiopterin (BH4) is an essential naturally occurring cofactor for several enzymes, including the three isoforms of nitric oxide synthase (NOS), phenylalanine hydroxylase, tyrosine hydroxylase, and tryptophan hydroxylase. In the context of cardiovascular research at The University of Texas at Arlington, BH4 is being studied for its role in endothelial function and its potential to mitigate oxidative stress-induced vascular impairment. BH4 is a critical regulator of endothelial nitric oxide synthase (eNOS) coupling; a deficiency or oxidation of BH4 leads to eNOS uncoupling, where the enzyme produces reactive oxygen species like superoxide instead of the vasodilator nitric oxide. This mechanism is hypothesized to contribute to the impaired skin blood flow and increased cardiovascular risk observed in certain populations, such as African Americans. The UTA study (NCT04154306) utilizes local BH4 supplementation via intradermal microdialysis to investigate whether restoring BH4 levels can improve microvascular function and reduce oxidative stress.
