Overview

Tetrahydrocannabinol (THC) and cannabidiol (CBD) are small molecule cannabinoids being investigated in a Phase 2 clinical trial by the University of California, San Diego, for the treatment of complications associated with HIV infection. THC acts primarily as an agonist at the cannabinoid receptors CB1 and CB2, while CBD has a more complex pharmacological profile, including negative allosteric modulation of CB1 and interaction with various non-cannabinoid receptors. The study (NCT05514899) evaluates the effects of these compounds on the gut microbiome, the endocannabinoid system, and neuroinflammation in people living with HIV, aiming to improve gut barrier function and reduce chronic inflammation. The trial uses a randomized, crossover design to compare the independent and combined effects of oral THC capsules and CBD oral solution.
