Tetrofosmin is a small molecule diphosphine ligand used in conjunction with the radioisotope technetium-99m to form the radiopharmaceutical technetium Tc 99m tetrofosmin. This compound is primarily used for myocardial perfusion imaging (MPI) in nuclear medicine to assess blood flow to the heart muscle under both stress and rest conditions. The mechanism of action involves passive diffusion of the lipophilic cationic complex into viable myocardial cells, where it accumulates in proportion to regional blood flow and binds mainly within mitochondria. Areas with reduced uptake indicate compromised perfusion due to coronary artery disease or infarction. Tetrofosmin was developed as an improved agent for cardiac imaging due to its favorable biodistribution and rapid clearance from non-target tissues[2][5][6][7].
