TF10 is a trivalent bispecific monoclonal antibody developed by Immunomedics (now Gilead Sciences) for the pre-targeted radioimmunotherapy (PRIT) and imaging of pancreatic cancer. Engineered using the proprietary "Dock-and-Lock" (DNL) platform, TF10 consists of two humanized Fab fragments that target the PAM4 epitope of the Mucin-1 (MUC1) glycoprotein and one Fab fragment that binds to the synthetic hapten histamine-succinyl-glycine (HSG). The PAM4 epitope is highly specific to pancreatic ductal adenocarcinoma and is rarely expressed in normal tissues or other malignancies. In a clinical pre-targeting protocol, TF10 is administered intravenously to localize at the tumor site; after the antibody has cleared from the blood, a small radiolabeled peptide (such as IMP288) carrying the HSG hapten is administered. This approach allows for the concentrated delivery of therapeutic radionuclides (e.g., Yttrium-90 or Lutetium-177) or imaging agents to the tumor while significantly reducing systemic radiation exposure and off-target toxicity.
