Overview

TG4010 + nivolumab + chemotherapy is an investigational combination regimen studied primarily in advanced non-small cell lung cancer (NSCLC). **TG4010** is a therapeutic cancer vaccine using a recombinant Modified Vaccinia Ankara (MVA) viral vector engineered to express the tumor-associated antigen MUC1 and interleukin-2 (IL-2). **Nivolumab** is a monoclonal antibody immunotherapy that blocks the immune checkpoint programmed death-1 (PD-1), enhancing T-cell responses against tumors. **Chemotherapy** in these studies usually refers to standard first-line regimens, most often platinum-based doublet therapy such as cisplatin/gemcitabine or cisplatin/vinorelbine. This multi-modal approach aims to both stimulate an anti-tumor immune response (via the vaccine), remove immune inhibition (via checkpoint inhibition), and directly cytotoxically target tumor cells (via chemotherapy). Preclinical and clinical studies have established the rationale for this combination, with Phase 2 studies demonstrating safety and potential efficacy, especially in patient subgroups with specific immune biomarkers (e.g., low triple positive activated lymphocytes, TrPAL). As of the search cutoff, a Phase 2 trial investigating TG4010 + nivolumab + chemotherapy in NSCLC is ongoing, and FDA IND approval has been granted for study in this setting[1][4][5][6].
