TGFβi NK cells are an investigational cell-based immunotherapy developed by Nationwide Children's Hospital for the treatment of recurrent or refractory high-grade gliomas and other malignant pediatric brain tumors. These are universal donor (UD) natural killer (NK) cells that have been ex vivo expanded and engineered to express a dominant negative TGF-β receptor (dnTGFβRII) which lacks the intracellular kinase domain. This modification renders the NK cells insensitive to the immunosuppressive effects of TGF-β, a cytokine prevalent in the brain tumor microenvironment that typically inhibits immune responses. By bypassing this suppression, the cells maintain their cytotoxic potential against tumor cells. The therapy is delivered locoregionally via an Ommaya reservoir or a programmable ventriculoperitoneal (VP) shunt directly into the tumor cavity or resection site.
