TGN142 is a recombinant human butyrylcholinesterase (rBChE) developed by TGN Biotech. It is produced using transgenic animal technology, specifically expressed in the milk of transgenic pigs. TGN142 is designed as a bioscavenger for the treatment and prophylaxis of organophosphate poisoning, including exposure to nerve agents (such as sarin, soman, and VX) and organophosphorus pesticides. The mechanism of action involves the stoichiometric binding of the butyrylcholinesterase enzyme to organophosphate molecules, neutralizing them in the bloodstream before they can reach and inhibit acetylcholinesterase at the neuromuscular junction and in the central nervous system. This prevents the cholinergic crisis associated with organophosphate toxicity. TGN142 represents a scalable approach to producing high-purity human BChE, which is otherwise difficult to obtain in therapeutic quantities from human plasma.
