TH-302 (evofosfamide) + dexamethasone + pomalidomide is an investigational combination regimen for the treatment of relapsed or refractory multiple myeloma. Evofosfamide (formerly known as TH-302) is a hypoxia-activated prodrug of the DNA alkylator bromo-isophosphoramide mustard, selectively activated under low oxygen conditions typical in tumor microenvironments. Pomalidomide is a potent immunomodulatory agent that exerts direct tumoricidal effects on myeloma cells by promoting proteasomal degradation of Ikaros and Aiolos transcription factors, modulating immune responses, interacting with bone marrow stromal cells, and inhibiting angiogenesis. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties. The combination aims to exploit synergistic anti-myeloma activity through complementary mechanisms: targeting hypoxic tumor regions (TH-302), immune modulation and direct cytotoxicity (pomalidomide), and broad suppression of inflammation and lymphocyte activity (dexamethasone). This regimen has been studied in phase 1/2 trials for patients with heavily pretreated relapsed/refractory multiple myeloma[3][6].
