thalidomide + clarithromycin + dexamethasone is a combination therapy investigated for the treatment of multiple myeloma. This regimen, sometimes referred to as the TCD or BiTD regimen, leverages the synergistic effects of its three components. Thalidomide is an immunomodulatory drug (IMiD) that binds to the cereblon E3 ubiquitin ligase complex, leading to the degradation of Ikaros (IKZF1) and Aiolos (IKZF3) transcription factors, which are essential for myeloma cell survival. Dexamethasone is a synthetic glucocorticoid that induces apoptosis in plasma cells by activating the glucocorticoid receptor. Clarithromycin, a macrolide antibiotic, is included not for its antimicrobial properties but for its ability to increase the serum concentration of dexamethasone by inhibiting the CYP3A4 enzyme and for its potential direct immunomodulatory and anti-angiogenic effects.
