Overview

**thalidomide + epoetin alfa** is a combination of two pharmaceutical agents used together primarily in hematologic disorders, notably myelodysplastic syndromes and idiopathic myelofibrosis. Thalidomide is an immunomodulatory drug with anti-inflammatory and anti-angiogenic properties, acting mainly by modulating cytokine production and inhibiting tumor necrosis factor-alpha and the VEGF pathway. Epoetin alfa is a recombinant form of human erythropoietin, a hormone that stimulates erythropoiesis (red blood cell production) by acting on erythroid progenitors in the bone marrow. The combination has been studied for its potential to improve anemia in patients unresponsive to either agent alone; however, studies indicate that sequential or combined use may benefit only distinct subpopulations rather than providing a synergistic effect. Notably, the co-administration increases risk of thromboembolic events and must be managed with caution.
