Theophylline + bambuterol is an investigational oral combination therapy being developed by the Norwegian Armed Forces Medical Service for the potential treatment and prevention of high altitude pulmonary hypertension (HAPH) and hypoxemia. The combination consists of theophylline, a non-selective phosphodiesterase (PDE) inhibitor, and bambuterol, a long-acting β2-adrenergic receptor agonist and prodrug of terbutaline. Theophylline provides anti-inflammatory effects and mild bronchodilation, while bambuterol acts as a pulmonary vasodilator to reduce the elevated pulmonary artery pressure typically associated with high-altitude exposure. This combination is intended to address the physiological challenges of low-oxygen environments. A Phase I randomized, double-blind crossover trial in healthy volunteers evaluated the safety and pharmacokinetics of the combination, finding it generally safe with no significant drug-drug interactions, although common side effects included tremors and hypokalemia.
