Overview

This combination therapy, consisting of antioxidants (ascorbic acid and vitamin E) and B vitamins (thiamine and riboflavin), was evaluated by the National Institute of Allergy and Infectious Diseases (NIAID) in a pilot clinical study. The primary objective was to prevent the recurrence of hyperlactatemia in HIV-infected patients who were restarting nucleoside reverse transcriptase inhibitor (NRTI) therapy. Hyperlactatemia in this population is frequently associated with NRTI-induced mitochondrial toxicity. The B vitamins (B1 and B2) act as essential cofactors for mitochondrial enzymes involved in the Krebs cycle and the electron transport chain, thereby supporting cellular energy metabolism. Vitamins C and E serve as antioxidants that scavenge reactive oxygen species, potentially mitigating the oxidative stress and tissue damage caused by mitochondrial dysfunction.
