thiotepa + busulfan + fludarabine + cytarabine is a reduced-intensity conditioning (RIC) regimen being investigated for use in allogeneic hematopoietic stem cell transplantation (allo-HSCT) for patients with relapsed or refractory peripheral T-cell lymphoma (PTCL). This combination therapy, evaluated in a Phase II clinical trial led by Xianmin Song, MD, at the Shanghai General Hospital, aims to provide effective lymphodepletion and myeloablation with reduced toxicity compared to standard high-dose conditioning. The regimen's mechanism involves the synergistic action of DNA alkylating agents (thiotepa and busulfan) which cause DNA cross-linking, and antimetabolites (fludarabine and cytarabine) which inhibit DNA synthesis and repair mechanisms. The primary goal of this specific RIC protocol is to improve progression-free survival (PFS) and overall survival (OS) in the PTCL patient population.
