Overview

The TEAM regimen is a myeloablative conditioning therapy consisting of thiotepa, etoposide, cytarabine, and melphalan, administered prior to autologous hematopoietic stem cell transplantation (auto-HSCT). Developed as an alternative to the carmustine-containing BEAM regimen, TEAM replaces carmustine with thiotepa to mitigate pulmonary toxicities such as interstitial pneumonia and fibrosis. Thiotepa's ability to effectively cross the blood-brain barrier makes this regimen particularly advantageous for patients with non-Hodgkin lymphoma (NHL) or Hodgkin lymphoma who are at high risk for central nervous system (CNS) relapse. The combination utilizes multiple mechanisms of action, including DNA alkylation (thiotepa and melphalan), topoisomerase II inhibition (etoposide), and antimetabolite-mediated DNA polymerase inhibition (cytarabine), to achieve deep cytoreduction and marrow ablation.
