Overview

thiotepa + pirtobrutinib + sintilimab is a combination therapy being investigated by Zhejiang Cancer Hospital for the treatment of primary or secondary central nervous system lymphoma (CNSL), particularly in frail or relapsed/refractory patients. This regimen, often referred to as TPS, combines three distinct therapeutic approaches: thiotepa, a cytotoxic alkylating agent that cross-links DNA to inhibit cell division; pirtobrutinib, a next-generation, non-covalent (reversible) Bruton's tyrosine kinase (BTK) inhibitor designed to overcome resistance seen with covalent BTK inhibitors; and sintilimab, a humanized monoclonal antibody targeting the PD-1 receptor to restore T-cell mediated anti-tumor immune responses. The combination aims to leverage the blood-brain barrier-penetrating capabilities of thiotepa and pirtobrutinib alongside the immune-modulating effects of sintilimab to improve outcomes in difficult-to-treat CNS lymphomas.
