This investigational treatment regimen combines intrathecal thiotepa with the standard Stupp regimen, which consists of oral temozolomide and radical radiotherapy. Developed and investigated by the Second Affiliated Hospital, School of Medicine, Zhejiang University, this approach is specifically designed for newly diagnosed glioblastoma (GBM) patients whose tumors have invaded the ventricular system or spread to the meninges. Thiotepa is a small molecule alkylating agent that works by cross-linking DNA strands, thereby inhibiting DNA synthesis and cell division. In this protocol, thiotepa is administered intrathecally (directly into the cerebrospinal fluid) to bypass the blood-brain barrier and target tumor cells in the leptomeningeal space or ventricles. Temozolomide, another small molecule alkylating agent, is administered orally as part of the concurrent and adjuvant phases of the Stupp regimen. This combination therapy is currently being evaluated in a Phase 2 clinical trial to improve progression-free and overall survival in this high-risk glioblastoma population.
