THN102 is an investigational small molecule combination therapy developed by Theranexus, consisting of the wake-promoting agent modafinil and a low dose of the antiarrhythmic agent flecainide. Flecainide acts as a glial connexin modulator, specifically targeting astroglial gap junctions (Connexin 43 and Connexin 30) to enhance the wake-promoting and procognitive effects of modafinil. The drug was primarily developed to treat excessive daytime sleepiness (EDS) associated with narcolepsy and Parkinson's disease. While it showed promise in preclinical models and early clinical trials, a Phase 2 trial in narcolepsy patients did not meet its primary endpoint of superiority over modafinil alone, although it demonstrated a satisfactory safety profile.
