Overview

THN391 is a potential first-in-class, high-affinity, humanized monoclonal antibody designed to selectively block fibrin-mediated neuroinflammation without interfering with coagulation pathways. It targets the cryptic P2 epitope (γ377–395) of fibrin, inhibiting the inflammatory and oxidative responses triggered by deposited fibrin at sites of vascular dysfunction in the brain and retina. This mechanism aims to prevent neuronal damage associated with chronic neuroinflammation in neurodegenerative diseases such as Alzheimer’s disease and retinal conditions like diabetic macular edema. Preclinical studies have shown that blocking fibrin-induced inflammation can protect against vascular and neuronal degeneration. In Phase 1a clinical trials, THN391 demonstrated a favorable safety profile, dose-proportional pharmacokinetics supporting monthly dosing, no impact on coagulation or hematological parameters, and no anti-drug antibody response[1][3][4][5][6].
