THR-149 is a synthetic bicyclic peptide that acts as a highly potent, reversible inhibitor of human plasma kallikrein (PKal), a serine protease involved in the kallikrein-kinin system (KKS). By inhibiting PKal, THR-149 reduces the release of bradykinin and downstream effects such as increased retinal vascular permeability and edema. It is being developed primarily for the treatment of diabetic macular edema (DME), particularly in patients who have shown suboptimal response to anti-VEGF therapies. The drug has demonstrated favorable safety and tolerability profiles in clinical trials, with no serious adverse events reported. Its mechanism offers an alternative pathway for DME management beyond VEGF inhibition[2][5][9].
