THR-18 is a novel peptide-based biological drug developed to enhance the safety and efficacy of tissue plasminogen activator (tPA) therapy in acute ischemic stroke. Its primary mechanism is inhibition of plasminogen activator inhibitor 1 (PAI-1), which allows it to bind tPA and reduce adverse effects such as intracranial hemorrhage, without impeding tPA’s clot-dissolving activity[1][2][6]. Preclinical studies have shown that co-administration of THR-18 with tPA improves cerebral blood flow, reduces infarct size and brain edema, and lowers the risk of hemorrhagic complications. Additionally, THR-18 alone has demonstrated neuroprotective effects in animal models[6]. The drug was initially developed by Hadassah Foundation and has reached Phase 2 clinical trials for acute ischemic stroke[2][3][4].
