Overview

A multi-drug immunosuppressive regimen combining Thymoglobulin, sirolimus, and mycophenolate mofetil used primarily in solid organ transplantation for induction and maintenance of immunosuppression. Thymoglobulin is rabbit anti-thymocyte globulin, a polyclonal antibody preparation that depletes circulating T cells and modulates immune activation pathways. Sirolimus (rapamycin) is an mTOR inhibitor that blocks IL-2-driven T-cell proliferation and inhibits mTORC1-mediated cell-cycle progression. Mycophenolate mofetil is a prodrug of mycophenolic acid, which inhibits inosine monophosphate dehydrogenase to block de novo guanine nucleotide synthesis, selectively suppressing proliferation of T and B lymphocytes. Together, the combination provides complementary mechanisms: rapid lymphocyte depletion and immunomodulation (Thymoglobulin), signal transduction blockade of lymphocyte proliferation (sirolimus), and antiproliferative inhibition of nucleotide synthesis (mycophenolate), aiming to prevent acute rejection and support graft survival.
